Commercial Fleet Service Request Checklist

A fleet service request is easier to review when it separates the urgent symptom from the broader maintenance picture. A short, consistent intake record helps Johnson understand the unit, operating need and desired timing before discussing scope or scheduling.

Unit identity

  • Fleet unit number.
  • Complete VIN.
  • Year, manufacturer and model.
  • Engine, transmission and axle details.
  • Mileage, engine hours and current location.
  • Mounted body or equipment manufacturer, model and serial number.

Operating context

Describe how the unit works: highway, local delivery, refuse, construction, municipal, agricultural, terminal, feed transport or another application. Note high idle time, stop-and-go duty, dust, temperature, heavy loads or other severe conditions. This context helps distinguish a generic schedule from the truck's actual use.

Current request

State what you want Johnson to review. Is the request preventive maintenance, a known repair, an observed symptom, a recurring concern or parts identification? If the unit is down, describe where it is and whether it can be moved safely. Do not drive or operate equipment when a safety concern or manufacturer instruction says it should be stopped.

Observed symptoms

Record when the symptom began and the conditions that reproduce it. Include warning lamps, displayed messages, temperatures, pressures, leaks, noise, vibration, smoke, changes in braking or steering, electrical behavior and recent work. Photos and short videos can preserve details that disappear later, but they do not replace safe inspection.

Maintenance history

Provide the most recent service date, mileage or hours, work performed, fluids and filters used, parts installed and any deferred items. Use the correct manufacturer maintenance schedule for the exact model, engine, configuration and duty cycle. A copied interval from another truck is not a controlled fleet record.

Parts and approval context

If the request includes parts, list the existing numbers, quantities, installed positions and deadline. State who can approve estimates or next steps and the best contact method. Never include payment-card information in the service request form.

For multiple units

Use one row per unit in your internal list. Do not combine VINs, symptoms and maintenance histories in a paragraph that makes it unclear which detail belongs to which truck. Prioritize units by safety, operational impact and desired timing, but wait for Johnson to confirm scheduling.

Submission is the start of review

A request does not reserve a service slot or authorize work. Johnson will review the unit identity, need, location, timing and available information before confirming the next step.
